Cerebrovascular accident (CVA) is the leading cause of death among chronic non-communicable diseases in Brazil, and the second leading cause of functional disability in the world. Therefore, actions aimed at changing this scenario in the different spheres of health and stroke care are crucial. Therefore, the Stroke Care Days aim to mix education actions in and for health, for different populations, prioritizing the objective of raising awareness about the different levels of care and attention to stroke. Here, we report on the main axes developed and the audiences involved, as well as the results obtained during the four (04) years of teaching, research and extension action linked to the Journey. Educational actions during all the days led to the construction and deconstruction of knowledge, which together, these two active actions lead and lead to the congruence between knowledge and dissemination of information about health and stroke disease. Measures and strategies such as this aim to change the scenario of the stroke patient's care route, increasing the number of recovered people and reducing the burden of sequelae of those affected.
